Alternative Care Application Process

Are you willing to care for a CHILD who is in need?

If you are interested in making a difference in a CHILD’s life, please contact the Alternative Care Program and ask for an application package that includes the following to be filled out:

  1. Application Form
  2. Medical Report Form
  3. Criminal Record Check
  4. Family Tree Chart
  5. Environment of Care
  6. Statement of Confidentiality
  7. Consent to Release Information
  8. Home Study

The agency will request the following to be included in your application:

  • Three References
  • A photocopy of your vehicle insurance and identification cards (Driver’s License, Status Card, BC ID Card)
  • Your Family Month Schedule

An overall report will be done, as part of the approval process and you may support your application with copies of certificates, diplomas and support letters.

If you are approved, the next step to help a CHILD will be:

There will be a home visit arranged to complete your process, the Alternative Care worker will go over the reporting procedures, expectations and the signing of an Alternative Care Agreement.
